Athletics at DPMHS

Students enrolled at Daniel Pearl Magnet High School are eligible to participate in the Athletics program at Birmingham Community Charter High School (BCCHS). Birmingham has a long history of strong sports teams and are well known for their extensive offerings. In order to participate in Interscholastic Sports for any LAUSD school, the following must be met:
 
1- A minimum 2.0 Grade Point Average
 
2- A physical from a doctor. Must be in the District form. An office stamp and a signature is required. Physicals must be completely filled out including blood pressure, pulse and vision screenings. Doctors must individually check each required segment (straight lines cannot be drawn down on the sheet).
 
2A- If the student-athlete has asthma, a separate Asthma Action form must be filled out by the doctor. 
 
3- Student-Athletes must show proof of health insurance. If the student does not have insurance, it can be purchased through K and K insurance

Athletic Clearance Checklist
 
  • Create an account on homecampus.com/login
  • Account is under parent’s name
  • Two emergency contact numbers provided Parent and Student signed digital signatures
  • Proof of insurance uploaded into system
  • All sections complete and accurate
  • Confirmation page printed and signed by parent
  • Submit confirmation page to Mr. Fernandez. (Parent Consent)
  • Upload copy of physical & turn in original to Mr. Fernandez
 
If you require a physical because you have not gotten one or the form you are using is old (New form as of 7/30/21) please go to the West Oaks Urgent Care Center at 20181 Saticoy St.  Winnetka Ca. (818) 709 - 5700.  This is where Birmingham is sending their players.  It is not normally a long wait and it costs about $20.  The LAUSD does not claim support for this institution and is not affiliated with them.
