REQUIRED FORMS

Every student/athlete must have a current Sports Physical on file with the School Nurse.  The attached form must be used.  Original form needs to be turned in to the Daniel Pearl School Nurse to be cleared by LAUSD.  Take a picture and Upload to your HOMECAMPUS.COM to be cleared by Birmingham.

 

An emergency card is to be turned in to the coach when you show up for Tryouts.
